When an individual is transferring to or from another county, activities that are related to the transfer are allowable under assessment, care planning and referral/linkage. It is important that separate codes for different activities between the counties are billed.
The Case Note Guide states under Activities that can be billed:
- Consultation (other SSAs within the county or county to county transfers, Supervisor, Director, other agency/providers’ staff)
- County to County transfers – Ensure separate codes for different functions between counties are billed
