Streamlined Process for Live-In Caregiver!
The Ohio Department of Medicaid (ODM) introduced an online request form for an exemption to the Electronic Visit Verification (EVV) program for when the direct care provider is a resident of the same household as the Medicaid waiver recipient.
You will need to upload documentation for address verification (for example, utility bill, driver’s license, tax return, W-2, etc).
Prior to submission of the form, the provider must ensure the direct care worker and the Medicaid recipient records are in the EVV system
If you have already submitted the paper form, you do NOT need to submit an online request.
