
New Error Codes in the eMBS Weekly Provider Reports Requiring Electronic Visitation Verification
Attention: Provider Agencies and Independent Providers
DODD added two new error codes for claims requiring Electronic Visitation Verification (EVV).
The existing codes below have been displayed in the EVV weekly reports since they were published. These codes indicated if visits/units were found in EVV to match the claims being processed. Note that Error 66 meant no visit information was found, and Error 67 indicated that a visit was found, but not enough units were documented to cover the billed claim.
(66) NO EVV TIMESHEET DATA FOUND
EVV is an electronic-based system that verifies when provider visits occur and documents the precise time services begin and end. The Ohio Department of Medicaid (ODM) has contracted with Sandata Technologies LLC to deliver the EVV system. eMBS has not found any data in the Sandata EVV system to verify the submitted claim. Claims with these service errors will deny effective service dates starting October 1, 2025.
(67) EVV UNITS ARE INSUFFICIENT
EVV is an electronic-based system that verifies when provider visits occur and documents the precise time services begin and end. ODM has contracted with Sandata Technologies LLC to deliver the EVV system. eMBS has found an insufficient number of units in the Sandata EVV system to verify the submitted claim. Claims with these service errors will deny effective service dates starting October 1, 2025.
Two new error codes have been added to help explain why visits were not found. Error 70 indicates that no EVV account was found to match the provider on the claim, and error 71 indicated that the provider’s connection to the individual in the claim was not found in EVV. This should help the provider diagnose and resolve some issues around claims that do not pass the check with EVV.
(70) Provider not found in the Sandata EVV system

ODM has contracted with Sandata Technologies LLC to deliver the EVV system. A provider account matching the Provider Medicaid number with DODD was not found. Please set up an EVV account associated with the Medicaid number on file with DODD. Claims with these service errors will deny effective service dates starting October 1, 2025.
(71) PROVIDER/INDIVIDUAL CONNECTION NOT FOUND IN EVV
EVV is an electronic-based system that verifies when provider visits occur and documents the precise time services begin and end. ODM has contracted with Sandata Technologies LLC to deliver the EVV system. Please update your account in EVV to connect the individual to the provider. Claims with these service errors will deny effective service dates starting October 1, 2025.
These errors displayed starting in Pay Cycle AUG24D, which was picked up for processing on August 20, 2025.
