Non-Medical Transportation Documentation: New for 2025

Here is a quick overview of the fundamentals of documentation for non-medical transportation!

Documentation requirements include:

• Mode of NMT service (per trip, per mile or commercial)
• Date of service
• License plate number of vehicle used to provide service
• Name of individual receiving service
• Medicaid # of individual
• Name of provider
• Contract # of provider
Origination and destination points of NMT provided
• Times the trip started and stopped
• Written or electronic signature of the driver
Names of individuals in the vehicle during any portion of the trip

In addition, for per mile:

• Total number of miles in each distinct commute (beginning/ending odometer readings)
• Number of individuals (ratio/group size)

In addition, for per trip, to/from competitive integrated employment (CIE):

• The name and address of the individual’s employer
• The number of miles in each one-way trip by recording beginning and ending odometer readings

In addition, for commercial:

• Type (ie; bus, transit, taxicab, etc)
• If date of service is not applicable, date of purchase of fares, tokens, or vouchers
• Receipt showing amount paid

Non-medical transportation shall be provided pursuant to an ISP conforming to rule 5123-4-02 of the Administrative Code.
