TCM Tip – Reason, Action, Progress/Outcome

When writing your case notes, always remember to follow R.A.P.

  • Reason: Why did the contact occur?
  • Action: What action was taken during the contact?
  • Progress/Outcome: What was the result or outcome of the contact?

TCM case notes should clearly document significant contacts related to an individual’s services. Ensure your notes provide a clear summary, so anyone reading them can easily understand what occurred and how continuity of care was maintained throughout the service provision.
