TCM activities reimbursable under the assessment category include the following:
- Activities involving coordination with therapists or appropriately qualified individuals to determine an eligible person’s needs for medical, educational, social, and other services, including those related to technology and employment.
- Activities that determine an eligible individual’s eligibility for Home and Community-Based Services (HCBS) waiver programs or Medicaid.
- Activities related to evaluating and recommending an eligible individual’s ongoing need for HCBS waiver programs and estimating associated costs.
