TCM activities reimbursable under the care planning category include the following:
- Ensuring the active participation of the eligible individual.
- Collaborating with the individual and others to establish goals and course of action to address needs.
- These activities include the development, monitoring, and ongoing revision of the Individual Service Plan (ISP).
