TCM activities reimbursable under the monitoring and follow-up category include the following:
- Conducting activities and maintaining contact necessary to ensure that the Individual Service Plan (ISP) is effectively implemented and addresses the needs of the eligible individual.
- Reviewing trends and patterns identified in reports of unusual incidents and Major Unusual Incidents (MUIs), and incorporating prevention plans into ISP amendments as needed.
- Coordinate with service providers to ensure services outlined in the ISP are delivered as planned.
- Performing activities and maintaining contact to ensure that guardians and eligible individuals receive timely and appropriate communication and notifications related to unusual incidents and MUIs.
