Examples to Avoid:
- “Went to the individual’s home”
- “Drove to the individual’s home”
- “Traveled to the individual’s home”
- “On my way to meet with the individual”
Better Alternatives:
- “Met with the individual in their home”
- “At the individual’s home”
- “Meeting at the individual’s home”
Remember: The case note should reflect allowable service time, not travel time.
