When documentation includes multiple entries for the same individual, same task, and same day, each note should clearly reflect R.A.P. (Reason, Action, Plan) for that specific time documented. If the SSA is returning to a previously interrupted task, the entry should not repeat identical language. Instead, it should document what was completed during that specific documented period of work and any new progress made at that point in time.
Case notes should demonstrate updated progress, not repeated content, in order to accurately reflect service delivery throughout the day.
