
To ensure Medicaid reimbursement for Targeted Case Management, the following must be included in your case notes:
- Date of Service: Include the month, day, and year the service was delivered.
- Individual’s Name: Clearly document the legal name of the individual receiving the service. Name should match Medicaid card.
- Service Description: Provide a detailed but concise description of the service delivered.
- Location: Indicate where the service activity took place, either within the case note or in a coded system with an accessible key.
- Time of Service: Record both the start and end times of the activity. Ensure accurate AM/PM is selected.
- Provider Identification: Include the name and signature of the provider for each entry, along with a legend linking the name, title, signature, and initials to the entry. This is typically completed through electronic signature in software and policy.
Accurate and complete documentation is essential to avoid compliance issues and ensure proper payment.
